Our Wellness Process

We take a functional approach to balance your health and well-being.
With the services we provide, Sarah and her team can customize your plan of care. We’re committed to doing things differently because YOU are worthy of that.

You will have an in-depth meeting with a nurse practitioner to review your medical history, symptoms, issues. The focus of the first step is to gather information, meet the team, and determine any testing that might be necessary.

You will work with a lab for blood work. We believe in a "test don't guess" philosophy, so the lab work provides valuable information to inform your personalized plan of care.

You will meet with your nurse practitioner again to review all your lab results and options for treatment plans. Together, you will determine what the best path forward is for you in your wellness journey. This is called the Plan of Care appointment.

You will begin your treatment plan, and how that treatment plan looks depends on you. Your individual lab results, your needs and goals, and your comfort level are all part of designing what will work best for you.

Treatment plans could include a combination of supplements, peptides, bio-identical hormones, customized IV vitamin infusions, or Ozone and UltraViolet Light Therapy. Your plan is personalized to you.

Wellness Access Membership

Total Functional Health & Wellness Membership- $325 per year

All new patient wellness consultations include a 3 month complimentary membership for our total functional and holistic wellness program. Membership will auto renew unless canceled at least 14 days prior to your renewal date. 

Any client working with a nurse practitioner or nurse on our fully-personalized, functional and holistic wellness program must subscribe to the Total Functional Health & Wellness Membership. 

You are NOT required to have an Active Wellness membership for any of our walk-in wellness services such as IV Infusions/ Injections or Aesthetics.

Labs

Northwestern Labs is our laboratory partner that has off-site locations and does accept insurance. If you choose to send your lab work through insurance, the billing will come directly from Northwestern (HealthLab), so you will need to contact your insurance to determine costs. DTE Wellness cannot determine the cost of labs processed through insurance providers nor can we determine the costs of these off-site lab services. 

We can provide an exact cash pay option with a discounted lab fee paid directly to DTE Wellness at the time of the service. There are never any further bills or costs related to that cash pay lab draw. 

Wellness terms and conditions

Our Total Wellness Provider Access Fees allows you priority access to your provider and medical team for personalized care.

Real health care takes time. It takes a knowledgeable provider who actually listens to you. It requires testing a range of levels within your body and having someone who can understand the results.  It requires a provider who educates you and empowers you to make your own health decisions.

A lot of health care isn’t like that. Time with providers and blood work tests are controlled by what insurance companies will pay. Even other wellness places put clients into a one-size-fits-all protocol determined by the business, not by the client’s individual health needs.

What Is Functional Medicine?

Our bodies and their systems (organs, muscles, bones, etc.) are masterpieces of design. Every part has its specific functions as well as an interrelationship with the whole. When you experience symptoms, it’s your body’s way of telling you that something isn’t working at its full function. Our role is to work with you to determine what’s not functioning, why, and how we might assist you in restoring your health.

We use a holistic, supportive, patient-centered approach to healthcare that benefits patients of all ages and conditions. As functional medicine practitioners, we consider the web of interactions that occur among the following:

-Nutrition
-Digestive function
-Detoxification function
-Energy metabolism
-Hormonal function
-Structural function
-Immune system function/inflammation

We clarify the extent of any dysfunction by intensive listening (taking your history), physical examination, and laboratory tests. Once we have identified the root causes of your symptoms, we make recommendations. This might include specific treatments and therapies, which could include a combination of peptide therapies, IV infusions, or bioidentical hormones. We strategically plan which treatments are suited to your symptoms and work with you on what treatments you are comfortable receiving. There may also be recommendations for dietary changes, nutritional supplementation, herbal medicines, exercise, stress management, chiropractic treatment, mind-body medicine, and pharmaceutical use. We consider ourselves to be your partners in improving your health, so you are never alone in figuring out how to address your needs and meet your goals.

We also use functional medicine to assess your risk of developing certain health conditions and to prevent chronic illnesses from occurring. In this way, we move from a downstream (reactive) approach to an upstream (proactive) approach. We can help you determine what factors are “pushing you into the river” in the first place by identifying your susceptibilities. One of our newest approaches is functional genetic testing. Although you cannot change the “hand of cards you have been dealt” genetically, we can assist you in “playing your hand” in a more effective way. This approach offers significant promise regarding prevention of cancer, heart disease, and osteoporosis, as well as successfully addressing a number of common maladies.

What does the Wellness consultation include?

Your first consultation you will have approximately an hour with NP Sarah Godsave. She will listen to your concerns and health history. 

What should I bring to my wellness consultation?

Here is a list of what you should bring to your initial visit:

1: Completed Wellness Assessment Form (Located in your patient portal)
2: List of all medications and supplements that you are currently taking
3: Bring any recent labs or imaging that you have had done in the past 2-3 years
4: Feel free to bring a friend or family member with you if you would find that helpful.

After my consultation what does the process look like?

You will meet with Sarah again, this time to review all your lab results and options for treatment plans. Together, with Sarah, you will determine what the best path forward is for you in your wellness journey. This is called the Plan of Care appointment. You will design your personalized treatment plan. Your plan could include a combination of supplements, peptides, bio-identical hormones, customized IV vitamin infusions, or Ozone and UltraViolet Light Therapy. It is individualized to your needs, your body, and your comfort level. 

Prescription Refills

We would like to provide you with prescription refills as quickly as possible. Our office receives many portal requests from patients and pharmacies on a daily basis for refills.

Please submit all requests 3-5 business days prior to your prescription needing refilled. 

If you have not been seen for over three months, you may need to schedule an appointment prior to getting your refill.

Cancellation Policy

If you need to cancel or reschedule your appointment, please notify us at least 48 hours prior to the scheduled appointment. We reserve the right to charge half of the service amount if notified 24-48 hours prior to the appointment or the full service amount if notified 24 hours or less.

Late Policy

Because we take time with every client, we schedule very carefully and make every attempt to stay on schedule. That’s why if you are late for your appointment by more than 15 minutes, we will have to reschedule your appointment. Individuals who are late more than three times will only be allowed to schedule on the same day. Please call us if you are running late so that we are able to accommodate others who need to be seen. We know life happens! But we hope this policy allows for some flexibility without sacrificing our commitment to honoring the time all our clients spend with us.

Billing/Payments

Down to Earth Wellness Center does not bill insurance. Payment is expected at the time of service. We will provide you with a receipt for services, which you may submit to your insurance plan for reimbursement.

We are not a participating provider in Medicaid or Medicare and we have opted-out of Medicare. This means your appointment cannot be submitted to Medicare for any coverage.

How do I get started as a functional medicine client?

Please fill out our New Patient Wellness Consultation. After you fill out that form a member of our wellness concierge team will contact you to set up your appointment, discuss your health goals, and answer questions about our office and services. We want to make sure that you feel comfortable with our services and how our offerings will best support you on your journey to optimal health.

How will I be reminded of my appointment?

You will receive a reminder of your scheduled appointment that requests you confirm the date and time.

You will also receive text reminders as your appointment approaches, and if the appointment is not confirmed, a wellness concierge will reach out to you by phone as well. Text reminders will come from the number 888-330-3024; that number cannot receive text responses.

To cancel or reschedule your appointment, you must call Down to Earth Wellness Center at 317-978-0280 or email info@dtewellnesscenter.com 24-48 hours in advance per the cancellation policy.

Cancellation Policy

Walk-in Wellness/ Aesthetic Services

If you need to cancel or reschedule your appointment, please notify us at least 24 hours prior to the scheduled appointment. This includes all IV Therapy, Injections, HydraFacials, and Chemical Peel appointments. We reserve the right to charge the full service amount if notified 24 hours or less.

To ensure prompt service, appointments are always recommended; however, walk-ins are gladly accepted based on availability.

Provider appointments and procedures

If you need to cancel or reschedule your appointment, please notify us at least 48 hours prior to the scheduled appointment. This includes any office visit with a provider and bioidentical hormone pellet insertions. We reserve the right to charge the full service amount if notified 48 hours or less.

Weight loss clients must cancel appointments 24 hours prior to your scheduled appointment or may be subject to a $40 cancellation fee.
